Insomnia, fatigue common in people with cancer (Reuters)

NEW YORK (Reuters Health) ? More than half of cancer patients may suffer symptoms of insomnia during treatment, and for some, sleep problems can persist for months afterward, according to a new study.

The findings "point to the fact that sleep, including insomnia symptoms, are a really big problem for cancer patients," said Carol Enderlin, who studies sleep in breast cancer patients at the University of Arkansas for Medical Sciences in Little Rock.

"Many of them may think this is just something they have to deal with," Enderlin, who was not involved in the new research, told Reuters Health.

But the message for patients, she said, is "to be aware of sleep and the importance of sleep, to report changes in sleep to your healthcare provider before they become severe (and) to not be afraid to bring them up."

Canadian researchers, led by Josee Savard of the Laval University Cancer Research Center in Quebec, asked close to 1,000 patients getting surgery for cancer whether they had trouble falling asleep at night or staying asleep. Then, they regularly followed up with the same patients to see if their sleep and sleep symptoms changed over the months after treatment.

Patients were between 23 and 79 years old, and most had early-stage cancer, including breast and prostate cancers.

At the time of treatment, 59 percent of patients reported symptoms of insomnia, and about half of those were severe enough to qualify as insomnia syndrome, a collection of persistent symptoms such as requiring more than half an hour to fall asleep at least three nights per week. That rate was three times higher than insomnia syndrome rates in the general population.

One and a half years later, 36 percent of the participants reported insomnia symptoms.

While patients were generally less likely to report insomnia as time went on, one in seven developed symptoms for the first time in the months after surgery.

In another study published concurrently in the Journal of Clinical Oncology, Julienne Bower of the University of California, Los Angeles, and colleagues found that close to two-thirds of recently treated breast cancer patients suffered from fatigue and poor sleep quality.

It's only in recent years that sleep has been recognized as a problem for cancer patients, researchers said.

"There was a shift in attention a decade ago as cancer survivors were living longer," Bower said. "Oncologists were beginning to notice that they actually were having these persistent side effects and symptoms, and fatigue has probably been the most common."

Staying active during cancer treatment and maintaining a positive outlook could help ward off some of those symptoms, Bower said.

Her research team has been testing the effectiveness of yoga as a treatment for fatigue and tai chi for insomnia. Cognitive behavioral therapy, which has helped other people with insomnia, might also work for cancer patients with fatigue or insomnia, she said.

"If you try to change the way you're thinking about your fatigue, try to change your behaviors, that may be helpful," Bower said. "You can usually tolerate having some fatigue while you're going through treatment, but you really don't want to have it for the next 10 years," she added.

Enderlin said she hopes doctors and nurses will start routinely asking cancer patients about sleep and addressing sleep-related symptoms.

"When people are faced with stress, when they are faced with challenges, they do much better on a good night's sleep," she said. "It's very important, never more so than with cancer patients."

Study finds clear association between clinic accessibility and emergency room visits

(Edmonton, Canada) Access to health care and the usage of emergency departments are popular topics in the news.

David Jones, a graduate student in the University of Alberta's School of Public Health, along with Linda Carroll, professor in the School of Public Health, and Leonard Frank, executive director of the Leduc Beaumont Devon Primary Care Network, recently completed a study that examined whether or not there was a clear association between the number of visits to the emergency department and the availability of an after-hours care clinic in Leduc, Alberta.

Jones took into account 28 months worth of data with a population size of about 20,000 people. The statistics took into consideration non-urgent and semiurgent patient visits to the Leduc Hospital emergency department for 14 months prior to the opening of an after-hours clinic and 14 months after the clinic opened.

Based on this study, "there was a 40 per cent reduction in semi-urgent patients using the emergency department," said Jones.

Semi-urgent patients are classified, for example, as those that have suffered minor trauma, or who have abdominal pain or acute joint swelling. Non-urgent patients typically suffer from ailments such as nasal congestion or chronic low back pain, says the Canadian Triage and Acuity Scale.

The after-hours clinic is operated by the Leduc Beaumont Devon Primary Care Network, a joint partnership between participating family physicians in the area and Alberta Health Services, said Frank. "Primary care networks are based on the idea of finding local solutions to local needs and this clinic is a tremendous example of that."

The family physicians who work at the Leduc Hospital noted that increasingly patients were being seen in the emergency departments who could have been better served in a primary care setting such as the after-hours clinic. "Having a more appropriate use of resources was important to the physicians in trying to best serve their patients," said Frank.

Jones agrees. "These two types of patients, it has been argued in the literature, can be well served in a physician's office rather than an emergency department, but they contribute to emergency department overcrowding."

The after-hours clinic provided access to a physician between the hours of 6? p.m., Monday through Thursday.

"This study provides further evidence that offering these services can positively impact emergency department use, and while we have to be careful about population size and intervention, we did see a definite reduction in emergency room visits," said Jones.

Jones is currently working with the Leduc Beaumont Devon Primary Care Network and the Health Quality Council of Alberta to examine his findings further. The study was recently published in the Journal of Primary Care and Community Health.

Obama to speak on economy in wake of downgrade (AP)

WASHINGTON ? President Barack Obama plans to deliver a statement about the economy as the nation grapples with the first downgrade of U.S. debt in the country's history.

Obama will speak at 1 p.m. EDT from the White House.

Standard and Poor's on Friday said it was downgrading U.S. debt because the rating agency lacks confidence that political leaders will make the choices needed to avert a long-term fiscal crisis. Congress and Obama recently struck a deal to avoid a government default and to cut the deficit, although the deal was not as broad as the president wanted.

Obama is also expected to speak about Saturday's shoot-down of a helicopter in Afghanistan in which 30 U.S. troops were killed ? the deadliest incident for U.S. forces in the decade-long war.

The reason that social media marketing is so successful in today?s business environment is, as already outlined, down to the consumer mindset, rather than business strategists. Since the internet (and digital marketing in general) became common place, people have been working hard to avoid unwanted adverts. Things such as popup blockers and spam filters are rendering direct online advertising campaigns (such as email marketing) costly and useless.
The alternative is to identify what people are doing online, and adapt a business strategy to fit in with it, rather than sitting outside it. Fortunately, this isn?t hard to achieve. ?The key is to take a closer look at what makes social media so popular.

The whole point of social media is that it?s social. People are interacting in more varied ways and more frequently than they used to ? the only difference being that is mainly online. This is the reason that inbound marketing has the advantage over traditional outbound marketing.

The rebirth of word of mouth

Consider a customer who is looking to buy a new TV. They know they want a particular size of screen and they have a budget. They may have researched a few places, looking at brochures or magazines, for example. They tell their friends, who now know what they?re looking for. That person instantly has all the knowledge and experience of their friends at hand and can begin to explore options that they may not have thought about before. Now they know about that independent shop in the city centre where the owner is friendly, the prices are clear, and the salespeople don?t try to force a sale. This word of mouth has cost nothing and is likely to have secured a sale. This is the mentality that businesses need to consider and understand.

People generally appreciate being allowed to research and purchase at their own pace, which fits perfectly within a social media marketing strategy. You no longer have to try and convince people that they want a product, but simply offer it to those who have already decided it is for them. If you make this a focal point of your content and ensure that your product is visible and available all the time, you reduce most of the effort of trying to time launches, doing surveys and other activities. ?You won?t need to find someone to buy from you. They will find you.

This process is far less pushy. Rather relying on marketing activities like those mentioned earlier, your company?s business strategy relies on online word of mouth, with the potential for thousands of unpaid people to essentially volunteer to advertise your company. Even better, their words will be far more trusted and will not be filtered out like an unwanted email marketing campaign. It?s important to note that this process does not sit exclusively within the ?what to buy? group. As well as allowing people to share ideas on what to buy, you have the added benefit of continued discussion, both before and after any sales. While you may already have a technical support department, the great thing with social media marketing is that you?ll draw in people who enjoy helping. Simply set up a forum and watch customers share ideas, tips and solutions.

Obama seeks to reassure faith in US credit (AP)

WASHINGTON ? Eager to calm a nervous nation, President Barack Obama on Monday dismissed an unprecedented downgrade in the nation's credit rating, insisting investors will stand by the United States even as stock markets plunged. Obama said Washington can fix its ills by showing more political will.

"Markets will rise and fall, but this is the United States of America," Obama said. "No matter what some agency may say, we've always been and always will be a triple-A country."

Investors did funnel money on Monday into Treasurys, a sign of confidence in the United States as a safe long-term investment even after Standard & Poor's had dropped the U.S. credit rating down a notch. But the broader story was far more worrisome: stock markets kept tumbling over concerns about the weakening U.S. economy and the debt crisis in Europe.

The Dow Jones industrials plummeted to its worst drop since December 2008.

For Obama, a president seeking a second term from voters desperate for better times, the pressure for results is intense.

He is the first president to have a credit downgrade come on his watch. And whether blaming him is fair or not ? he actually pushed for the type of deal that might have prevented a downgrade ? presidents are always accountable.

After saying nothing about the downgrade all weekend, Obama sought Monday to use it as leverage against a Congress whose members are on an August vacation. He said a downgrade ought to compel a smart compromise from the bipartisan committee of lawmakers that will soon be tasked with shaping up to another $1.5 trillion in difficult deficit reduction.

Obama said he would offer his own recommendations, although the White House suggested that would likely mean ideas Obama has already presented in recent weeks. Obama on Monday said Congress should ask wealthy Americans to pay more in taxes and should make adjustments in programs like Medicare. Both ideas face fierce political opposition.

"I assure you, we will stay on it until we get the job done," the president assured. But Congress remains in divided political hands, limiting Obama's ability to keep that promise.

Heading into a campaign-style economic tour before going on his own vacation, Obama's overall rating hovers below 50 percent in most polling. He is on far more perilous ground when it comes to public views on the key issue for voters ? his handling of the economy ? where his approval rating is under 40 percent in most major recent surveys.

Obama's aim is to keep heat on Congress to enact concrete measures, separate from the grueling debt debate, which could help people in the short term.

He pressed lawmakers to extend a payroll tax cut and unemployment benefits in September as a way to "put money in people's pockets and more customers in stores."

On a jittery day for the financial world, it fell to Obama to deal with a downgrade that S&P had warned for weeks would come if Obama and Congress failed to agree on a major debt-reduction package. The agency was dissatisfied with the deal lawmakers reached last week to cut more than $2 trillion from the debt over 10 years.

The administration has derided the downgrade as having no economic basis. S&P, though, has little faith in Washington's ability to overcome its partisan woes on the debt.

"We didn't need a rating agency to tell us that the gridlock in Washington over the last several months has not been constructive," Obama said.

S&P has dropped the government's rating to AA+ from the top rating, AAA. The agency attached a negative outlook that means the rating could be lowered again.

Asked under what scenario the United States could regain its AAA rating, David Beers, head of sovereign ratings at S&P, told reporters on Monday, "We don't anticipate a scenario at the moment where the United States could quickly return to AAA."

S&P officials said that five countries including Canada and Australia have lost their AAA ratings from S&P and then regained them. The shortest time that it took a country to regain an AAA rating was nine years and the longest time was 18 years.

Wall Street had its first chance to react to the downgrade on Monday. The Dow fell below 11,000 for the first time since November. The sharp drop extended Wall Street's almost uninterrupted decline since late July, when the Dow was flirting with 13,000.

Republican candidates hoping to challenge Obama next year have placed blame for the downgrade on Obama, tying it to his larger economic agenda.

Former Massachusetts Gov. Mitt Romney, who leads the GOP field in fundraising and early polls, said the downgrade was a "deeply troubling indicator of our country's decline under President Obama." Rep. Michelle Bachmann, R-Minn., who has shown strength in the early voting state of Iowa, accused Obama of "destroying" the U.S. credit rating.

Obama calmly sought to dismiss all the talk of a dent to U.S. credit.

"Our problems are eminently solvable," he said. "We know what we have to do to solve them. Our problem is not confidence in our credit. The markets continue to reaffirm our credit as among the world's safest. Our challenge is the need to tackle our deficits over the long term."
